NCCN Endorses ctDNA-Guided Treatment Strategy for Muscle-Invasive Bladder Cancer

The National Comprehensive Cancer Network (NCCN) has updated its Bladder Cancer Guidelines to recommend consideration of personalized circulating tumor DNA (ctDNA)-based molecular residual disease (MRD) testing after radical cystectomy in patients with muscle-invasive bladder cancer (MIBC). The guideline recognizes tumor-informed ctDNA testing as a valuable tool for identifying patients at higher risk of recurrence and helping guide decisions regarding adjuvant immunotherapy.
The recommendation is based on results from the landmark Phase III IMvigor011 trial, which used the Signatera™ ctDNA assay to identify patients who benefited from adjuvant atezolizumab after becoming MRD-positive. This represents a major advance toward personalized postoperative cancer care, with ctDNA-guided treatment now receiving the NCCN's highest Category 1 recommendation for eligible patients.
